Job Summary: The primary focus is to complete admissions for new clients entering the program on Thursdays. It is the responsibility of the LPN to ensure we are admitting clients into the program who are medically clear to participate in the program. Manage and ensure all medications are being provided safety. The LPN will be expected to work closely with licensed counselors, all staff members, related service providers, families and other agencies as appropriate.
Essential Job Duties and Responsibilities:
- Intake: covid rapid testing, drug testing, health history questionnaire, baseline vitals.
- Medication Reconciliation on all medications brought on admission, corresponding with pharmacy to ensure accurate dosing and enough medications are on site.
- Completes Medical Administration Record (MAR) for each client upon admission
- Provides education to clients/staff regarding prescriptions and proper administration
- Medication administration.
- Meets with clients regarding medical concerns
- Correspondence with client physicians or walk-in clinics to when medication evaluation is required.
- Medication administration auditing and MAR review.
- Orders medical supplies for facility
- Keeps accurate documentation for clients and care in EMR
- Communicates changes in client medical information to counselors and other staff as appropriate
- Assists clinical team in reviewing new client applications prior to intake
- Provides one health related session to the clients weekly.
- Conducts required annual med administration training to staff
- Reviews/revamps intake policies as necessary
Qualifications/Requirements:
- Graduate of an accredited school of nursing
- Maintains current LPN license
- Minimum of 1 year experience in skilled nursing duties
- Knowledge regarding the nursing process and its application, including the assessment, planning, implementation and evaluation of nursing care
- Must possess excellent communication skills with clients and co-workers
- Must have critical thinking skill with self autonomy.
- Familiarity with addiction and mental health
- Must have knowledge of Indigenous history and culture.
Physical Requirements:
- Ability to perform the essential physical qualifications of the position. Must be able to lift a minimum of 30 pounds
